DETA Ballarat Fees

DETA’s fees follow the Government fee schedule for Allied Health Professionals and payments can be made by credit card, debit card (No American Express accepted) or cash on the day. Full fee payment is required on the day of appointment and clients will be provided with an invoice for Medicare rebate.

If you have been referred via a GP Enhanced Primary Care Plan (EPC), Medicare will pay you a rebate of $52.95 providing you have not exceeded 5 EPC visits per year.

Concession rates are available upon presentation of current and valid Healthcare or Pension card in the name of the client at every visit.

Melton & Bacchus Marsh Bulk Billing Clinic Fees

The Melton and Gell ST clinics opperate on a Wednesday/Thursday and the Bacchus Marsh clinic on a Monday/Friday. Bookings are limited and will only be Bulk Billed providing a current Enhanced Primary Care Plan is presented.

Enhanced Primary Care Plan (EPC)

If a current Enhanced Primary Care Plan is completed, the allocated number of visits for Diabetes Education will be Bulk Billed for Melton and Bacchus Marsh clients. However, if Medicare Australia rejects the claim, you will be required to pay an out of pocket fee of $55.00.

Bacchus Marsh Specialist Centre Bulk Billing Clinic Fees

The Bacchus Marsh clinic opperates on a Monday/Friday and bookings will only be Bulk Billed providing a current Enhanced Primary Care Plan is presented.

Enhanced Primary Care Plan (EPC)

If a current Enhanced Primary Care Plan is completed, the allocated number of visits for Diabetes Education will be Bulk Billed for Bacchus Marsh. However, if Medicare Australia rejects the claim, you will be required to pay an out of pocket fee of $55.00.

Private bookings can also be arranged at all clinic locations.

Department Of Veterans Affairs (DVA)

No charge - at all locations as long as a current DVA D904 referral is provided. Please note: D904 referrals are valid for a period of 12 months.

CANCELLATIONS
We ask that you provide at least 24 hours’ notice for all cancellations or rescheduling of appointments. Less or no notice will result in a fee charge of $52.95
